In August 2011, 23-year-old Shane O'Farrell was killed in a hit and run while out cycling near Carrickmacross, Co. Monaghan.
The driver, who had 42 convictions, was in breach of multiple bail conditions and, according to Shane's mother Lucia O'Farrell, "should have been in custody."
Earlier today, Lucia met with Garda Commissioner Drew Harris to raise a number of concerns about her son's death.
These concerns were previously debated in the Dáil, which last year voted in favour of a commission of investigation. However, the government has so far failed to set this up.
