A number of mental health services in the West are still being run on guidelines dating back to the 1980's.
A HSE report published this weekend shows that some psychiatric units in Galway and Roscommon are basing their practices on the 'planning for the Future' - A document first released in 1984.
Teams carrying out the study also noted that key information wasn't available to them.
