Australia's Prime Minister has acknowledged the nation's security system failed to keep track of the gunman responsible for siege at a Sydney cafe in which two hostages died.

Tony Abbot has also promised an investigation into why self-styled cleric Man Haron Monis wasn't on any terror watch list.

New South Wales Police Commissioner Andrew Scipione (pron: skippy-oh-knee) says he's thanked the tactical teams and officers involved in the operation.
