An investigation's been launched by Gardaí and the HSE into the death of a woman following an incident at St Colmcille's Hospital in Dublin.
The 68 year old woman was transferred to St Vincent's Hospital on the 17th of May and passed away two days later.
Two separate investigations have been launched into the circumstances surrounding the incident and her death.
