An investigation has found that children were left in a foster home for years, in spite of allegations of sexual abuse against one of the foster family.
The report by RTE's Investigations Unit centres on a foster home in the west of Ireland.
It found that a girl in foster care made a credible abuse allegation against a teenage family member in 2007.
In spite of this, two other children - a boy and a girl - were left in the care of the family.
Four years later, the second girl also made abuse allegations.
She was removed from the home, but the boy remained with the family for another year.
The boy's birth mother told RTE she was only informed of the allegations five months after they were made:
