An investigation is underway after a three-year-old boy died after becoming trapped in a lift shaft at a building in Galway city.
The Gardai and The Health and Safety Authority have launched separate inquiries into the accident, which happened at around 3 o'clock this afternoon at the Hynes Building on St. Augustine Street.
The boy was with his Nigerian born mother and two other siblings at the time of the accident.
Witnesses at the scene said the child's mother was inconsolable, and his siblings in deep shock following the incident.
