HIQA publishes terms of reference for Portlaoise investigation

The Health watchdog is to investigate the patient safety culture at Portlaoise Hospital - and examine whether the HSE's governance structure is supporting services at the hospital or not.

HIQA has published the terms of reference of its investigation into the Midlands Regional Hospital, after the deaths of five babies since 2007.

A report by the Department of Health found that the maternity service at the hospital was not safe or sustainable - and an interim management team was put in place.

The Health Information and Quality Authority has begun its investigation into the hospital - and will be examining the extent of serious adverse incidents at the hospital, the HSE's actions, inactions and response to the incidents and the relationship between the HSE and the patients and families.

HIQA also said it would investigate whether Portlaoise hospital and the HSE had learned lessons from the investigation into Galway University Hospital following the death of Savita Halappanavar.
