The HSE is overhauling its services in order to help patients suffering from multiple health conditions.
The Health Service says it's the biggest ever overhaul of GP and community care services - and will see new groups set up to oversee GP, Social and Primary Care.
It's intended that these so-called 'Community Healthcare Organisations', will take over the running of primary care facilities when the HSE is eventually disbanded.
