The mother of a baby boy who died shortly after being born at the Midlands Regional Hospital in Portlaoise says she spent years terrified of having another baby.

Baby Joshua Keyes died shortly after he was born on October 28th 2009.

Today - the HSE issued a 'comprehensive' apology to his parents after a report into his death found significant failures in care provided to him and his mother.

The report was published at the request of his parents and it made 23 recommendations which have been implemented at the hospital.
