A major overhaul of the child protection system is needed to ensure the safety of children at risk.
An audit of Garda cases where powers to remove a child from their family were used has found a number of serious failings.
The report has issued 17 recommendations including improved training for Gardai, better cooperation between agencies and improvements on the PULSE computer system.
