The Child and Family Agency has apologised for significant failings in the care of a teenager who spent much of his life in state care before dying of a drug overdose.
A report into the death of 19 year old Danny Talbot found there were serious weaknesses in management and accountability around the case, and considerable gaps between his needs and services offered by the state.
ItÂ’s one of four reports published today by the National Review Panel of the Child and Family Agency into the deaths of young people known to child protection services.
