After an hour advice to 'stay put' was scrapped

The advice to residents caught up in the Grenfell Tower fire to stay put, had "substantially failed" more than an hour before the policy was scrapped.

Newly released reports also describe numerous design and construction problems which increased the risk of flames spreading.

The fire was initially blamed on a faulty fridge-freezer, but new analysis says that's not certain. 

Counsel to the official inquiry into the disaster, Richard Millett QC, says there are serious questions to answer.
