An internal review will be carried out into the death of a woman after she gave birth at St Luke's Hospital in Kilkenny.

36 year old Tracey Fitzpatrick, died after giving birth to a boy on Easter Monday.

Miss Fitzpatrick was taken for surgery at the hospital, but it's thought she had already passed away.
